How do I provide my medical records?

To share your medical records you will need to do one of three things:

  1. The preferred method: provide your medical portal credentials (login and password) via the xINFORM portal. This is highly recommended, as it is the fastest and safest way. Or:
  2. Upload your medical records directly into xINFORM. Or:
  3. Give us the contact information of your treating physician(s) and institution(s) and we will request medical records on your behalf. Through this method, it may take between 2 to 6 weeks for us to receive your records.

What does my doctor/care team need to provide?

If you upload all the medical records we need, or share your medical portal credentials (MyChart or similar), your doctor/care team doesn’t have to provide anything – we can immediately start working on your cancer journey and options report. If not, and if requested, we will contact your doctor/care team for those records.

Will you contact my doctor/care team?

Upon enrollment, we reach out to your doctor/team to let him/them know that you are participating in an observational research protocol and will be receiving a list of treatment options attained from evidence-based research. 

What should I do if my condition changes and I need help again?

Once you register for xINFORM, we are here to support you long-term. If your condition changes and you need our assistance, just reach out to us at [email protected]. We will work with you to obtain your most recent records as quickly as possible — if you provided your medical portal credentials, we will log in and download your recent records, or if you have them available, you can upload those records via your portal. Any additional information you can provide is also very helpful, such as when your next doctor’s appointment is or by when you need to make a decision regarding your treatment. We will then work diligently to generate an updated cancer journey and treatment options report for you.
